The gold standard for the evaluation of portal hypertension (PH) is the measurement of hepatic venous pressure gradient. However it is invasive and not useful in extrahepatic portal vein obstruction. Diagnosis of PH in children currently relies on evidence of its complications. Ultrasound is the first line diagnostic tool (spleen size, portal vein blood flow, portosystemic collaterals). Endoscopy is the standard for the diagnosis of esophageal varices.
