The effect of atrial natriuretic peptide (ANP) was investigated in seventeen primary hypertensive patients taking a constant diet. In this single blind placebo controlled, randomized study nine patients received atrial natriuretic peptide ANF 99-126, 150 μg over 30 minutes (protocol 1), while eight patients received ANF 99-126, 200 μg over the same period (protocol 2). ANP infusion increased urinary sodium (p < 0.001) and volume excretion. Blood pressure was significantly reduced (p < 0.05) during ANP infusion. The hypotensive and natriuretic effect obtained with 200 μg of ANP was significantly higher as compared to that obtained with 150 μg. This was related to the different plasma concentrations of ANP at the end of the infusion (464 ± 73 pmol/L in protocol 2 vs. 153 ± 131 pmol/L in protocol 1). One patient in protocol 1 and five patients in protocol 2 exhibited unwanted effects (dizziness, sweat, nausea, bradycardia and faintness).
