The appropriate diagnostic management of ACC (adrenocortical cancer) requires hormonal, radiological and pathological assessment. The endocrine evaluation (screening for hypercortisolism, hyperaldosteronism, androgen excess in women, estrogen excess in men) and imaging are also useful to monitor the tumor response to treatment and recurrences. The therapeutic management of metastatic patients is centered on systemic therapy including mitotane alone or in combination with chemotherapy, but mitotane is also used in adjuvant setting, because up to 80% of patients with nonmetastatic ACC show loco-regional or distant metastases after an apparent complete surgical excision.
