For sometimes, the principles of therapeutic education have been applied to treat adult patients with diabetes. Numerous studies have been conducted in this field, but few have monitored the adherence over time (in “chronicity”) of operators and patients. Although patients are trained in the management of the disease through therapeutic education, over the years the burden of chronicity often hinders the application of principles, skills and known behaviors. It is therefore necessary to monitor coping strategies, the relational dynamics of patients and family members and in particular the management of emotions and resistance to the management of insulin therapy. Furthermore, it is necessary to listen to their emotional and psychological fatigue and anxiety and offer effective help and support strategies. The same is true for health workers, who also suffer from the burden of chronicity. After reflecting on chronicity, coping strategies, goals of therapeutic education and limits in its implementation, this chapter describes the experience of “continuous” management of adult diabetes at the ASST Spedali Civili of Brescia. The paths dedicated to diabetic patients pay particular attention to the educational-experiential path of Mindfulness, which represents an alternative and appreciated approach based on the recognition of emotions, which is the first filter for their effective management and effective self-care behavior. The results of this pilot study, through the application of BDI, PAID-5 and SCL 90, are disclosed here. Regarding the management of chronicity by the health team, training and supervision groups have been implemented. In particular, data related to 69 supervisions, 43 medical interviews and 23 nurses based on communication skills and competencies were presented and evaluated. Clinical work has confirmed that an integrated approach, focused on the recognition of emotions and relational support is a fundamental element in the management of chronicity.
